Just like any other game, competitive CS:GO suffers from bugs from time to time. Fortunately, they rarely happen in the important matches, but now we have a different example. The ongoing PGL EU RMR tournament had teams replay the 40th round due to C4 not spawning. That happened in a match between Team Dignitas and Eternal Fire.

Teams were going forward face-to-face, however, overtime was unlucky for the T side. EF players had no C4 to plant in the key round. Their opponents took advantage of it, but later on PGL stopped the game and issued a 40-minutes investigation to sort things out.

After careful examination PGL decided to replay the round. This did change the outcome of the game, as Eternal Fire won the match with a scoreline of 22:20. They have moved to the 1:1 bracket, while Team Dignitas fell down and will be eliminated in case they lose one more game.
